Under current recommendations, only 1.4% of women under 50 receive referral for additional assessment, a figure that captures merely 4.4% of younger women who subsequently develop the disease. The researchers attribute this gap largely to the fact that three-quarters of women under 50 who develop breast cancer within ten years have no family history of the disease—the primary criterion in existing Nice guidelines.
While acknowledging the potential of multifactorial risk assessment tools, Nice stated that available evidence does not currently justify changing breast cancer screening guidelines. The organization noted concerns about the implications of expanded referrals, including potential strain on healthcare resources and possible anxiety for women undergoing additional testing. Lead researcher Dr Juliet Usher-Smith called for an NHS review of the criteria, while Cancer Research UK emphasized that any modifications must be equitable and minimize unnecessary worry among patients. Breast cancer remains a leading cause of death in women under 50 in the United Kingdom, though the majority of cases occur in older populations.
