The World Health Organization has released updated guidance discouraging the use of obesity medications and GLP-1 weight-loss injections such as Mounjaro and Wegovy in children under 10 years of age. While most of these medications lack licensing for use in this younger age group, some physicians have prescribed them off-label, and pharmaceutical companies are conducting clinical trials in children as young as six.
According to WHO recommendations, treatment approaches for young children should prioritize nutritional interventions and physical activity rather than pharmaceutical or surgical options. The organization indicated it does not oppose ongoing drug trials but expressed preference for lifestyle-based strategies. Limited evidence suggests that certain digital technologies, including activity trackers and interactive online games that promote physical movement, may provide some benefit.
Globally, approximately 170 million children and adolescents are classified as obese, with 70 million falling within the 5–9 age group. The prevalence of obesity in this younger cohort has increased substantially over recent decades, rising from 2% to 8% since 1990. Additionally, 35 million children under five were reported as overweight. Obesity carries significant health consequences, elevating the risk for numerous conditions including type 2 diabetes.
Dr Luz De Rigil, WHO’s Director of Nutrition and Food Safety, stated that the organization issues a strong recommendation against obesity medicines, bariatric surgery, and weight-management devices for children under 10. For older children and adolescents, such treatments may be considered appropriate when lifestyle approaches prove unsuccessful and individuals demonstrate adequate mental and physical readiness.
WHO officials emphasized uncertainty regarding the long-term implications of initiating GLP-1 medications in young children, particularly regarding treatment duration and the possibility of transitioning to dietary approaches later in life. The organization stated its intention to continue evaluating emerging evidence as the field evolves.
