Private care investment needs monitoring, regulator says

The Care Quality Commission has flagged potential risks from growing consolidation in England’s care home market, noting that the five largest operators have expanded their market share by 26% over the past two years and now control one in seven homes. The expansion has been fueled partly by investment from US property firms.

While consolidation can offer certain advantages, the regulator expressed concern that the trend could result in elevated costs and diminished service quality. The CQC also identified worrying patterns in home-based care services, where nearly a third of providers are serving four or fewer people, creating inefficiencies. Small care homes, traditionally valued for offering specialist and personalized support, have declined substantially over the last 14 years, particularly facilities combining care with nursing services, which have dropped by a quarter.

The CQC chief inspector noted that market reshaping is occurring “largely by accident rather than design,” driven by economic pressures and difficulties some local councils face in purchasing services for state-funded clients. The regulator emphasized the importance of a clearer strategic vision to ensure the market is structured around people’s needs rather than purely economic forces.

Funding fragmentation continues to complicate the sector. Some providers increasingly focus on self-paying customers, making it harder for councils to secure care for those dependent on state support. Care providers argued that smaller homes face mounting pressures from national insurance contributions, inflation, and energy costs, threatening their viability without improved funding.

The government acknowledged the inherited challenges outlined in the report and stated its commitment to overhauling the social care system, following the prime minister’s identification of social care as a key priority.
