Private care investment needs monitoring, regulator says

The Care Quality Commission’s annual report indicates that consolidation in the English care home market is accelerating, with the five largest operators increasing their market share by 26% over the preceding two years and now controlling one in seven homes. This expansion has been fueled by investment from United States property investors.

The regulator identified multiple structural concerns within the sector. At-home care services, which provide assistance with daily activities such as washing and dressing, have become increasingly fragmented, with nearly a third of providers serving four or fewer individuals. Meanwhile, small care homes—historically important for delivering specialist and personalized care—have declined significantly over the last 14 years, particularly those offering combined care and nursing support, which have decreased by a quarter.

The CQC described the market transformation as occurring “largely by accident rather than design,” driven by economic pressures and councils’ difficulties purchasing services for state-funded care recipients. The regulator emphasized that the funding system remains unstable, with individuals either paying privately or relying on state assistance. In some regions, providers increasingly prioritize self-funding clients, making it harder for councils to secure care for those dependent on public funding.

CQC chief inspector Chris Badger called for clearer strategic direction, noting that the government’s renewed emphasis on social care reform under current leadership was encouraging but that markets must be shaped “around what’s best for people.” Experts from think-tanks and care provider organizations stressed the need for adequate council funding, skilled leadership, and policy support for smaller providers. Care England representatives warned that without improved funding settlements and relief from rising operational costs, the trend of smaller homes closing would persist.
