Vivani Medical is advancing development of an experimental semaglutide implant designed to address a significant challenge in obesity treatment: patient adherence. Roughly half or more of patients discontinue GLP-1 medications within a year due to factors including injection fatigue, side effects, costs, and stigma, often resulting in weight regain. The Danish pharmaceutical company Novo Nordisk announced a new agreement with Vivani to evaluate its lead candidate, NPM-139, which would function as a maintenance therapy after patients initially achieve weight loss using existing injections or oral medications.

The implant is envisioned as a small titanium reservoir containing semaglutide that would be inserted under the skin, potentially in the upper arm, during an in-office procedure lasting several minutes under local anesthesia. The device features a specialized membrane with microscopic channels designed to release medication at a steady rate over several months, potentially reducing drug level fluctuations associated with periodic injections. Current development focuses on implants lasting six months, though the company aims eventually to extend duration to a full year, with dosing potentially needed only once or twice annually.

Several clinical and logistical hurdles remain before the implant reaches patients. Vivani recently received approval to begin its first human trial in Australia in mid-2026, with a phase one study enrolling approximately 20 participants. Medical experts emphasized the need for robust clinical data demonstrating safety, tolerability, and effectiveness compared to existing treatments. Cost and insurance coverage remain unclear, with uncertain impact on the anticipated $100 billion-plus GLP-1 market by the early 2030s.

Physicians noted both opportunities and concerns regarding adoption. The implant could benefit patients experiencing injection fatigue or difficulty with daily medications, and may help sustain the long-term treatment necessary for maintained weight loss and cardiovascular benefits. However, some patients may find implants uncomfortable, and procedural requirements could limit accessibility, particularly for those using telehealth platforms. Additionally, providers may require training in insertion techniques and complications management, potentially restricting availability to specialized centers.

Experts acknowledged the implant’s potential while cautioning that meaningful assessment requires human trial results. Pre-clinical rat studies showed approximately 20 percent weight loss over six months, compared to Wegovy’s documented 15 to 28 percent weight loss in human trials. Doctors emphasized that obesity treatment increasingly requires personalized approaches, with implants potentially serving as one option among multiple strategies for maintaining weight loss long-term.
